An effect of substance abuse is withdrawal, this is the phase in which the patient begins to feel the adverse effects of the substance. The receptors of the particular drug like opiate receptors or alcohol receptors are still being produced despite the patient not abusing the substance. This causes a neuro-chemical reaction that makes the patient crave for the drug.
